NfL is emerging as one of the most biologically informative markers of brain aging because it reflects cumulative axonal injury rather than a single disease pathway. The centenarian data is especially compelling since it reinforces that long term cognitive resilience appears to depend more on preserving neural integrity across systems than on amyloid burden alone.

I'm left wondering two things:

- If NfL predicts future brain disease or is indicative of current brain diseases.

- If NfL acting as a biomarker extends beyond the specific Japanese population the study was done in?

Hopefully more research is done to look into these things!
