Excellent review of a very important topic. Extreme keto diet coupled with forced lipid control via multi drug therapy can’t possibly be a sensible approach. I’m keenly interested to have more discussion of dynamic exercise triggered ketosis vs static ketosis in cognitively normal lean ApoE4 people. Dynamic ketosis provides signaling and autophagy and is much easier to reach. I gave up on trying to get nutritional ketosis through extreme carb restriction because it caused headaches, low energy, sleep disruption, and hypoglycemia with subsequent glucose counter regulation that was totally unproductive. Instead, I monitor glucose variation with a CGM, keep the variation low and easily transition into ketone levels of 1-4 with bouts of fasted exercise and heat (sauna, steam, elevating core body temp) while eating a moderate carb Mediterranean high fiber plant forward diet, keeping lipids in range without requiring medication.
