There is a subgroup of blood type A+ that experiences blood clotting and more coagulation/thrombosis with Covid vax and virus. I have not heard or seen any studies that stratify by blood type and APOE4 or comment if having A+ puts APOE4 individuals at even more risk of dementia. So many individual complexities. I would imagine if both result in some level of thrombosis, the vax would at least save the brain from worse effects of the virus itself.

The Radomyslsky paper on COVID-19 vaccines and dementia patients, cited a couple of times, is not an easy one to draw conclusions from. The mortality observation window for the vaccinated arm was the two weeks immediately following the second shot, and for the unvaccinated it was 400 days. The stated reason for this disparity was that it would show the vaccines at their maximum efficacy, although the references cited do not support this; they report the period of maximum efficacy as beginning at 14 days rather than ending there, and this is consistent with the public health messaging at the time. What the chosen two week observation period would do is maximise risk of healthy vaccinee bias, if clinicians were less likely to vaccinate people who appeared terminally ill. The Kaplan-Meier curves show the unvaccinated patients essentially all dead by day 400, suggesting that the clinicians administering the vaccines may indeed have been very good at identifying the terminally ill. The paper seems unfinished, and the various hyperlinks in the paper to supplemental material and appendices, which might have made the methods clearer, are all dead links.
